Guide · Sign-in sheet template

Where can I get a printable sign-in sheet template, and is there a better way?

Download the printable attendance sheet PDF below, or skip paper: a Sealique QR register lets participants check in by phone and exports a PAdES-sealed signed sheet.

A paper sign-in sheet is still the fastest thing to put on a table, so here is a clean one: name, organisation, phone or e-mail, time in, signature, with a header for the meeting title, date and venue. The second half of this page explains what the paper version cannot do, and how a QR register produces the same sheet, already signed and sealed, without anyone retyping names.

Key facts

  • The printable template is an A4 PDF, downloaded without an account, with twenty rows per page: name, organisation, phone or e-mail, time in and signature.
  • A Sealique QR register replaces the sheet: participants scan one code, fill the form and sign on their phone, with no app and no account.
  • The register closes automatically outside each day's opening window, so nobody signs in after the meeting.
  • The list exports to CSV (opens in Excel) and to an attendance sheet PDF sealed with a PAdES digital signature that anyone can verify online.
  • Neither the template nor the QR register needs a card; the organiser needs a Sealique account only for the QR version.

What is on the printable sign-in sheet?

A header with the meeting or training title, the date, the venue and the organiser; then a numbered table with columns for the participant's full name, organisation or department, phone or e-mail, time of arrival and signature. The PDF is A4 portrait, prints in black and white, and holds twenty rows per page; print as many pages as you need.

What can a paper sheet not do?

It cannot stop a colleague signing for someone else, it cannot record the time reliably, it has to be retyped into Excel, the handwriting is often unreadable, the sheet gets lost, and nobody can prove later that it was not altered. For a donor-funded training or a board meeting those gaps matter.

How does the QR register work instead?

Create the event in Sealique in one step, print the QR poster that the event page gives you, and put it at the entrance. Participants point the phone camera at the code, fill in the fields you chose (name, organisation, e-mail, your own custom fields) and sign with a finger. Attendance opens and closes at the times you set for each day; you can close early or re-open for latecomers. The list updates on the event page, downloads as CSV, and exports as an attendance sheet PDF sealed with a PAdES signature.

Which should I use for my meeting?

Use the paper template for a short internal meeting where the list is only a reminder. Use the QR register when the list is evidence: trainings reported to a funder, board or general assemblies, workshops with certificates, classes, or any event with more than twenty people where retyping names costs an afternoon.
